Required Intake and Consent Form

Clients:

  • If you are sick, or even suspect you may be slightly sick, please stay home. We are offering a “sickness cancellation policy”. You will not be penalized for cancelling your appointment due to illness. 

Studio:

  • We are limited as to the number of people we can accommodate in the studio at once. Please wait in your vehicle until your specific appointment time.

  • No walk-in appointments or inquiries at any time-please call. This includes the purchase of gift certificates and product, which needs to be done by appointment only. 

Health hygiene protocols:

  • Ongoing reinforcement regarding hand-washing and other preventative measures for cleaning door handles, common surfaces, front desk, equipment and supplies with disinfectants that are EPA –registered and labeled as bactericidal, virucidal and fungicidal per OSHA and CDC guidelines.
